I Request hereby to benefit from the services of banking information offered free of . charge by your Bank to its clients, through the telephone line (hereinafter the system) I declare that I have taken note of the rules and conditions of use of the system and of the functioning of the secret code, allowing access to the system, and its issuance in a sealed envelop. I undertake to preserve the confidentiality of the code attributed to me. Your Bank shall not be held responsible in any case, of the failure that may affect the system, and particularly the machines and/or the telephone lines which might lead to the communication of information relative to my account to third parties, or the transmission of erroneous information or the eventual reception of information related to third parties accounts, which I undertake not to reveal and to comply with the Bank secrecy regulations. I declare that the information obtained through the system do not have any power of proof and only the entries of Bank are conclusive.
It is understood that your Bank may modify the system at any time, or terminate it without prior notice. Your Bank may also confer the system against payment, which however must be subject to my prior approval.
